Registration Players (2010/2011 edition)

How To Register:

Register by mail

Print registration form and fill in all information.
Sign registration form.
Date registration form.
New players will need to provide a copy of their birth certificate.
Mail registration form, copy of birth certificate (if new) and cheque (do not mail cash) to:

A $50 non-refundable deposit is required with each completed registration form. You may pay the
outstanding balance in two installments of 50% each (see registration form for fees). Please remit all
cheques with the completed registration form.

1st cheque (deposit) must be current dated
2nd payment may be post dated for September 1, 2010
3rd payment may be post dated for November 15, 2010
