2011/2012 Player registration

How To Register:

Register in person at Conestoga Mall on Sept. 7 or 8 from 5:30 pm to 9:00 pm

·                          Print registration form and fill in all information.

·                          Sign registration form.

·                          Date registration form.

·                          New players will need to provide a copy of their birth certificate.

·                          Come see us at Conestoga Mall and bring a copy of birth certificate (if new) and cheque

·                          Or you can still mail it to:
